Vision/O&M
Vision Itinerant / Orientation & Mobility
Vision services are provided to students who meet the eligibility criteria for visual impairment and demonstrate an adverse effect on his/her educational performance. Ocular information is the starting point of the process. A functional vision assessment includes a review of the ocular information, observation, and consultation with the educational team. An assessment is then completed. If a student is found eligible, the student’s needs are addressed through direct service and/or consultation service. A certified teacher of the visually impaired provides training and collaborative support to the educational team. Specific areas of instruction may include, but are not limited to: braille reading and writing, large print materials, instruction in assistive technology for the visually impaired, compensatory strategies, daily living skills, and low vision devices.
Referral Process for Vision Services (FORMS)
The flowchart outlines steps from identifying concerns and obtaining medical documentation through assessment, eligibility, and support planning. Ocular Report Form
This form may be shared with a student’s optometrist or ophthalmologist to obtain current eye information. *For students with Cortical Visual Impairment (CVI), appropriate neurological documentation may also be considered.
When concerns arise, the process for obtaining a current medical eye report will begin.
After reviewing the report, the team will determine appropriate next steps, which may include a Functional Vision Assessment (FVA), Learning Media Assessment (LMA), Orientation and Mobility (O&M) assessment, or Cortical Vision Impairment (CVI) assessment.
Assessment results help the team identify accommodations, assistive technology, specialized instruction, and other supports. The IEP team will then consider eligibility and educational needs according to state and district procedures
